When you take your children for regular dental check up, problems such as crowding, missing and crooked teeth, problems associated with the jaw, dental hygiene can be detected early. It enables them to develop good oral health seeking behavior pattern. The earlier these problems are detected, the less difficult and less expensive the correction and treatment will be.

 Our preventive dental regime for kids includes the following

1. Fluoride therapy. fluoride is a natural mineral that builds strong teeth and prevent cavities. Its been an essential part of oral health treatment for decades. Fluoride supports healthy tooth enamel and fights bacteria that harm teeth and gums. Fluoride therapy is highly recommended if you are at a high risk of developing cavities. Its is also good for children who have had cavities affecting several teeth. Dentist provide professional fluoride treatment in the form of highly concentrated rinse, foam, gel or varnish. The treatment may be applied with a swab, brush, tray, or mouthwash. These treatments have more fluoride than what is in your water or tooth paste. You may be asked to avoid eating or drinking for about 30 minutes after the treatment so that the fluoride can be fully absorbed. Fluoride can not remove decay, but it can stop the decay from getting into the deeper layers of the tooth. Fluoride treatment benefits both children and adult. The earlier children are exposed to fluoride the less likely they are to develop cavities
2. Dental sealants. These are fillings that are placed in deep fissure (grooves) in the teeth especially the first permanent molars. Because these are among the first adult teeth to erupt into the mouth it is worth protecting them from cavity as they are supposed to serve the child for the rest of his/her life. Theses fissures(grooves) are usually not easy to clean
3. Night guards. The dentist may recommend night guards for children who play sport where a tooth can be damaged or knocked out

Because these materials are closely matched to the natural appearance of the enamel (the outermost layer of the tooth) of the tooth, it is usually preferred for the filling of cavities especially of teeth in the very visible areas of the mouth and also for the replacement of broken fillings. A direct composite filling will require a single appointment during which the decay is removed and the tooth restored. The dentist may give you local anesthesia to numb the tooth. Indirect composite, inlays and on lays are indicated for badly broken down teeth and also large cavities at the contact areas of the teeth. These may require two or more appointments

Root canal treatment also known as endodontic therapy is indicated when the pulp (the third and inner most layer of the tooth) of the tooth is infected due to extensive caries, fracture or trauma that leads to the death of the pulp. If a tooth in this state is left alone, it can lead to severe and significant pain and abscess formation which could easily become a dental emergency

Root canal treatment involves the removal of the infected and sometimes necrotic (dead) pulpal tissue. This is accomplished by the use of a combination of mechanical means (instruments that remove the debris inside the pulp chamber and root canals) and chemical methods (disinfecting solution injected into the tooth to kill bacteria)

To remove the infected and necrotic debris from the pulp and root canal (channels through which the pulp extends into the part of the tooth that is in the bone) manual files or electrically empowered instruments (rotary endodontic instruments)

ROTARY ENDODONTIC TECHNOLOGY

This involves the use of electric hand tools during endodontic therapy, it makes root canal procedure much faster than with conventional files. Other benefits are

• Single appointment completion of the procedure as opposed to the multiple visits of the manual procedure
• More comfort. Patients feel more comfortable
• Less unpleasant noises
• More precision
• Higher quality of treatment (more consistent)

SURGICAL ENDODONTIC THERAPY

Sometimes a nonsurgical root canal treatment alone will not be sufficient to save your tooth. The Endodontist (a dentist specialist in root canal treatment) will recommend a surgery

A surgical endodontic therapy may be required in the following instances;

1. If you have a persistent problem without any corresponding radiological evidence, the tooth may have tiny fractures or a canal that could not be detected during non surgical treatment. In this case surgery will enable the Endodontist to examine the entire root to find the problem and then fix it
2. Calcification. Calcium deposits may make a canal too narrow for the instruments used in non surgical root canal therapy to reach the end of the root. In instances like this endodontic surgery enable the Endodontist to be able to clean and seal the rest of the canal
3. When a tooth that has had a successful root canal therapy becomes infected
4. The presence of cyst that actively discharges through the root canal

SOMETIMES YOU HAVE TO LET GO

When a tooth has undergone excessive decay, or has been the cause of a spreading infection or there is crowding (too many teeth in the mouth), a dental extraction (which is the removal of the tooth) may be required. A dental extraction may be simple or surgical

A SIMPLE EXTRACTION

The dentist will give a local anesthesia to numb the area, and the tooth will be removed with the use of dental elevators and forceps without the need for an incision

SURGICAL EXTRACTION

if a surgical extraction is indicated, you may be given local anesthesia most of the time. The dentist will cut the gum by making a small incision and the bone around the tooth may removed and the tooth may be divided before being removed

The dentist may require dental radiographs to enable him assess the tooth very well.

Do well to inform the dentist if you have any medical condition for which you are undergoing treatment. It is important to follow the instructions that you will be given after the procedure

Usually if a surgical dental extraction is recommended then the benefits will outweigh the side effects and the risk involved. After an extraction, a clot will form in the socket and the socket will heal but if the clot is dislodged then the socket could become infected, and a condition called dry socket may result. If that happens the dentist will place a sedative dressing in the socket while it heals. And so it is encouraged the you come back for a review after an extraction

POST OPERATIVE INSTRUCTION (What to do after an extraction)

The following will aid you in recovering very quickly after an extraction and also to avoid any complications

DAY 1

• Bite hard on the gauze that the dentist will place over the socket for at least 30 minutes after which you can discard it
• Avoid spitting altogether and swallow your saliva and or mucus
• Take the prescribed medication after the gauze has been taken out after the 30 minutes
• Take a rest for the rest of the day and avoid jumping straight into your daily routine
• Avoid the use of straw and hot beverages
• Avoid brushing or rinsing your mouth for the rest of the day
• Avoid poking the socket to find out if there any remnant of the tooth
• Prop your head with pillow when you sleep
• Avoid chewing on the side of the extraction for the next two hours
• The dentist may instruct you to apply ice to the area for 10 minutes at time for the rest of the day

If you observe bleeding form the socket at home, sit up straight, roll a clean handkerchief over the socket and bite hard for 15 minute, if the bleeding does not stop, do repeat the above instruction for three more times and if the bleeding still persists contact the clinic immediately

DAY 2

• Resume your routine oral hygiene practices
• Add a teaspoon level of salt to 8 ounces of water and use it to rinse your mouth with emphasis on the area of the extraction. Repeat this at least three times a day for a week
• Continue with the prescribed medication
•  See the dentist for a review after a week

If there is persistent pain or welling before a week after the extraction do well to return to the clinic

A dental prosthesis is an artificial device that is used to replace or augment an intra oral (inside the mouth) defect such as missing teeth and missing soft or hard structure of the jaw and palate. Dental prosthesis is used for cosmetic and functional reason.

Dental braces are wire based appliance that the orthodontist (dental specialist) attaches to the permanent teeth in order to move and straighten them and also to close unwanted gaps in between the teeth to produce a pleasing smile and a very good bite (the relationship between the upper and lower teeth)

DENTAL BRACES ARE INDICATED TO CORRECT THE FOLLOWING

• To correct overcrowding
• To straighten crooked teeth
• To close too much spaces in between the teeth
• When the upper teeth bite in front of the lower teeth
• When the upper teeth almost cover the lower teeth

Suitable for both children and adults though adults may need to wear them longer than children.

Dental braces are very safe with very few side effects such as the loss of minerals from the enamel (outermost layer of the tooth) and a predisposition to decay and gum infection. These are easily overcome with proper oral hygiene practices

The procedure involves the following

• Oral examination. A thorough examination of the mouth and jaws are done and also to asses the oral hygiene status of the client
• Dental radiographs. To show all the teeth biting position
• Plaster molds. The orthodontist will use this to asses and evaluate the way your upper and lower jaws relate and the extend of tooth movement that may be required
• Probable tooth extraction. In instances of overcrowding there may be a need for the removal of some teeth before the treatment is commenced

The procedure is usually done in three phases

1. The initial placement of the braces. This may involve the use of fixed dental braces or the use of removable clear aligners. Fixed dental braces will involve the following components
   1. Brackets. Attached to either the front or back surface of the teeth. They are made of stainless steel, ceramics, gold or other materials
   2. Bands, these are ring-like and they encircle the molar teeth
   3. Flexible wires to connect the brackets and bands
   4. Elastic ties to secure the wire to the brackets

Removable clear aligners are used for patients who require minor corrections

1. Periodic adjustments. The orthodontist will need to adjust the braces by tightening the interconnecting wires, to achieve the desired effects
2. Retainers after the braces are removed. To prevent the teeth from shifting back to their old positions, a custom made appliance (retainers) are fixed unto the teeth to keep them in their new positions for a period of time

3. Veneers. These are custom made thin layer of tooth colored material that cover the front teeth to help you achieve a wonderful smile. Veneer are made of porcelain or composite and these are fixed on the tooth or teeth with strong dental adhesives. A composite may be built onto the tooth directly in the mouth (direct) or it may be done at dental laboratory and later fixed unto the tooth. They are able to restore the shape and the color of a chipped or discolored tooth. Also they can be used to close small gaps in between the teeth were orthodontic treatment may not be suitable.

Indications for a dental veneer

• Discolored teeth

the procedure may require the minimal removal of surface of the tooth, after which the veneer is place on the second visit. A temporary veneer may not be required.
